APPLICATION FORM
Please contact the school and attend a tour.
If you would like to schedule a student two day visit for the process of enrollment, you may choose to have your child accompany you on a tour. After visiting, you may wish to have them visit for a two day guest visit.
We will contact you to schedule your child's two day visit to our school after receiving a completed application. We may request additional information to help ensure every child who visits is able to be best supported at JLSA.
A $50 application fee is due upon scheduling a student visit.
All student visitors receive a letter confirming they were in attendance at JLSA for an educational evaluation.

Each prospective student’s application & student information form, and other pertinent information will be reviewed by staff. Basic assessments are performed by JLSA staff during the admission process. A variety of factors are used in consideration of admission, but perhaps none as important as the experience of spending time with the child as they visit the school for two days. It is our goal that the staff and families composing the school community will mirror the diversity of the community in which we live.
Acceptance to the school is in no way based on race, religion, ethnicity, gender identity, handicapping conditions, or socio-economic status.
